In certain cases with the addons Tab Mix Plus and Flash Video Downloader installed together, Fast Dial will not open in a new tab page. Here are a couple ways to fix that problem:

Update!! Issue is fixed by installing latest version of Flash Video Downloader.

1) Disable either:

TMP or Disable Flash Video Downloader

2) If you want to keep both extensions enabled, In TMP's settings enter the following url for the New Tab Page: chrome://fastdial/content/fastdial.html
