Fixed bugs:
- Middle click on a thumbnail was launching a thumbnail refresh instead of opening link in a new tab
- Clicking on a frame around thumbnail was bringing up "Set URL" dialog with all fields set to "undefined"

- Text "Refreshing..." now is displayed in place of the thumbnail
- Left click on thumbnail title opens "Set URL" dialog. Middle click on thumbnail title runs thumbnail refresh.

Added possibility to specify thumbnail size in preferences
