When uploading images, filenames containing ampersands (&) are not properly escaped, and lead to blank images. For example, try uploading a file with the name "Bill & Ben.png". The upload will appear to succeed (no warnings or anything) but the image will not appear, just a line where the zero-width degenerate image is. Changing to "Bill and Ben.png" will fix the problem.
